School-Located H1N1 Influenza Vaccination Event Resources

The Bureau of Immunization is pleased to bring the following resource to the attention of our colleagues and partners.

The Centers for Disease Control and Prevention has collected useful materials for those who are planning school-located H1N1 influenza vaccination events.  These materials include template letters to principals, healthcare providers, and parents; Vaccine Information Statements; and vaccination record forms in several languages, including Spanish.
