The FOMA 2013 Mid-Year
Seminar will be held September 20-22, 2013 at the Grand
Hyatt Tampa Bay in Tampa, Florida. Included in the program
will be the Florida mandatory hours of 1-A CME credits: Professional and Medical Ethics, Florida Laws and Rules, Prevention of Medical
Errors, and Federal and State Laws Related to the Prescribing of Controlled Substances. Also
being offered will be 2 hrs of Domestic
Violence. The Seminar will offer approximately 25 hours
of 1-A CME credits.
The Grand Hyatt Tampa Bay in Tampa is just five
minutes from the Tampa International Airport.
The Grand Hyatt Tampa Bay, Tampa's only four star, four
diamond luxury business resort, features three restaurants

Northern Italian cuisine and a penthouse view.

You will need to make your
room reservations early in order to protect the FOMA's
ability to block a sufficient number of room and adequate
meeting space. The cutoff date for hotel reservations
is August 28, 2013.

If you pre-register for the Seminar, your complete
packet, including name tags, tickets, programs, etc.
will be at the FOMA registration desk when you check
in. FOMA members receive a $25 discount if you
register prior to August 28, 2013.
The FOMA must be notified of any cancellation by
August 28, 2013, in order to provide a refund.
Cancellations received on or before this date will be
charged a $50 administrative fee. Cancellations
received after August 30, 2013 will not be refunded.
